The Building Community-Based Medical Homes for Children (BCMHP) Learning Session 1 was sponsored by the Illinois Chapter of the American Academy of Pediatrics (ICAAP), the Division of Specialized Care for Children (DSCC), University of Illinois at Chicago, & the national American Academy of Pediatrics. The learning session was supported by grants from the Michael Reese Health Trust, the Chicago Community Trust, and the Maternal and Child Health Bureau.
Learning Session Overview
The learning session was attended by invited medical home quality improvement teams from all over Illinois. Speakers presented on a number of topics, with a keynote presentation by Dr. Jane Taylor entitled ”Model for Practice Improvement and Testing Change”.
Available on this site are videos of eight presentations, including a parent-provider discussion panel, and documents made available to attendees of the learning session.

Speaker: Ms. Kathy Sanabria
Ms. Kathy Sanabria of the Illinois Chapter, American Academy of Pediatrics begins the learning session by introducing the day’s learning objectives and providing invited attendees an opportunity to introduce themselves. Newly formed quality improvements teams were welcomed to the Illinois Medical Home community.
Speakers: Ms. Tobias & Ms. Pazdioch
Ms. Tobias and Ms. Padzioch are parent partners and parents of children with special needs who shared recollections of their experiences seeking medical care for their children both before and after medical home experiences.
Speaker: Dr. Jane Taylor
Dr Taylor’s presentation is divided into three parts. Her discussion provides QI teams with diagnostic tools through which teams can identify, evaluate, and improve implementation of the medical home.
Speaker: Ms. Laura DeStigter
Ms. DeStigter describes the NCQA PPC-PCMH recognition program and explains the program’s recognition requirements.
Speaker: Ms. Suzi Montasir
Ms. Montasir introduces the AAP Center for Medical home Implementation website and demonstrates how the website may be used by QI teams.
Speaker: Ms. Roseann Slaght
Ms. Slaght provides an overview of the resources and services provided by the Family to Family Health Information and Education Center.
Speakers: Ms. Kathy Sanabria, Ms. Laurie Vlcek, & Ms. Pam Northrop
Ms. Sanabria, Ms. Vlcek, and Ms. Northrop explain what care coordination is, who needs care coordination, and how to provide it. The role of the care coordinator is explained in terms of the key characteristics of the role and through the experiences of professional care coordinators who implemented the medical home model in their practices.
