The 2011 Annual Medical Home Newsletter is here! The newsletter provides updates on what is happening to promote medical home implementation across Illinois. Many of the highlighted projects and programs are made possible by the generous support of the Chicago Community Trust, the Michael Reese Health Trust, and the federal Maternal and Child Health Bureau, to name a few. This year’s edition features information about a state implementation grant focusing on helping to transition youth with special health care needs to adult care, including a forthcoming Transitions Toolbox and a training for the inclusion of health goals in Individualized Education Plans (IEPs); new information about a training curricula to improve care coordination between primary care and Early Intervention; details on the Children’s Health Insurance Program Reauthorization Act (CHIPRA) demonstration grant; and updates on models of medical home implementation from Washington Pediatrics, Knoxville Pediatrics, Fantus Health Center, and La Rabida Premiere Kids Program.
