Improving Birth Outcomes | How to Get Involved

Grantees in this category were required to develop specific plans in the first year, and Illinois is focusing on improving birth outcomes, specifically reducing pre-term births.  Efforts will include publishing minimum care standards for prenatal and interconception care and measuring compliance; developing the prenatal electronic data set; improving care coordination for high risk women; connecting neonatal intensive care unit (NICU) graduates to medical homes and Early Intervention; and implementing and assessing the effectiveness of targeted prenatal and interconception interventions.

The project is seeking prenatal/obstetrical care providers to help define the prenatal care data set and who work in birthing hospitals and want to help test it.  We are also seeking primary care practices to participate in a new quality improvement initiative on care of the NICU graduate.  See the How to Get Involved section for more information.
