Improving Birth Outcomes | How to Get Involved
Efforts to establish Health Information Exchanges (HIEs) and enlist practices to use electronic medical records (EMRs) are ongoing through separate state and federal initiatives. The CHIPRA Demonstration Grant will connect with those activities to ensure that child health quality reporting, tracking, and quality improvement activities are integrated. Of particular note in this category, Illinois hopes to develop a mechanism to allow medical home providers to electronically make referrals to other medical and community service providers and receive care feedback reports. Illinois is also using this opportunity to develop a minimum data set describing women’s use of prenatal care services and prenatal risk and making it accessible to delivery hospitals at the time of delivery.
