Implementation activities for the Illinois CHIPRA project are underway.  ICAAP welcomes feedback and input from pediatricians, family physicians and others regarding each of the grant areas (e.g., How should status on the child health measures be delivered to providers?  What referral feedback is most important to receive?) .  In addition, ICAAP will be working with up to 200 practices in the medical home practice assessment and technical assistance (Category C) program.

Participate in the CHIPRA Demonstration Grant

If you are interested in receiving technical assistance and support to build a medical home as part of the IL CHIPRA Medical Home Quality Improvement Program, please complete the Practice Registration Form.
