Medical Home activities of the Illinois Chapter of the American Academy of Pediatrics (ICAAP) are supported through grants from the federal Maternal and Child Health Bureau, the Chicago Community Trust, and the Michael Reese Health Trust. ICAAP is partnering with many state and national organizations to disseminate important and timely medical home information. ICAAP also seeks to facilitate family-professional partnerships in primary care for Illinois children and families.
