The Building Community-Based Medical Homes for Children newsletter is here! It was published October 1, 2015. The newsletter is mailed to all ICAAP members. It is also available as a pdf. ICAAP has been working on several medical home initiatives over the past three years. A few have been completed and results are summarized in this seventh edition of the newsletter. Many other activities continue to support pediatricians, providers, and patients and families around the state. Sara Parvinian, MD, FAAP, ICAAP pediatrician leader, offers a thoughtful commentary on what “medical home” implementation has meant to her as a community pediatrician in Gurnee, IL as well as what it has done for patients.

The 6th edition of the Illinois Medical Home Annual Newsletter is now available. Newsletter articles include the evaluation results from the Building Community-Based Medical Homes for Children program, an update on the Illinois Children’s Health Insurance Program Reauthorization Act Quality Improvement Demonstration Project, information on how to enroll in ICAAP programs, and information on how to assess a practice’s “Medical Homeness.”

ICAAP’s projects “Coordinating Care between Early Intervention and the Primary Care Medical Home” and “Illinois Healthy Beginnings II” have worked to bridge systemic care for young children at risk for developmental delay. Both projects have developed tools and resources to improve care coordination and access by working with primary care providers, Early Interventionists, and families throughout Illinois. Two tools, “The Referral Fax Back Form” for Early Intervention Service Coordinators and “The Standardized Illinois Early Intervention Referral Form” for primary care providers, were developed in the Coordinating Care project and reached wider implementation with IHB2.

In November 2006 La Rabida Premier Kids began working with ICAAP in the Illinois Medical Home Project. During their four years working together, La Rabida’s Quality Improvement team made many achievements while becoming a medical home. These include greater parent participation and increased family-centered care; implementation of new care coordination tools for patients who see multiple providers; and greater clinic accessibility for patients. This partnership led La Rabida Children’s Hospital to apply for and win NCQA Recognition as a Patient-Centered Medical Home.

The 2011 Annual Medical Home Newsletter is here! The newsletter provides updates on what is happening to promote medical home implementation across Illinois. Many of the highlighted projects and programs are made possible by the generous support of the Chicago Community Trust, the Michael Reese Health Trust, and the federal Maternal and Child Health Bureau, to name a few.
